Congratulations and welcome to Franklin Pierce University. The purpose of this page is to ensure that all your forms and benefit information is available for you to complete and return to the Human Resources Department at your earliest convenience. As always, if you have any questions or comments, please contact at member of the Human Resources Team (603.899.4075) who would be happy to assist you. As you go through this page, you will note that you may not be eligible for all benefits immediately. Those benefits that require a waiting period are listed at the bottom. The following forms should be completed as soon as possible and returned to Human Resources. Benefit Election Form – please indicate if you are electing to take the benefit or decline the benefit. Sign, date and return. Standard Life Insurance form – please note, this coverage is paid for by the university. You will see a section for voluntary group life insurance – if you do not wish to take advantage of the guaranteed issue of up to $80,000 please leave this blank. If you are wishing to take voluntary group life insurance for yourself, spouse or dependent child, please contact a member of the HR Team who will assist you with premium rates. Flexible Spending Account – you are eligible to elect up to $3,000 for medical reimbursement or $5,000 dependent care. If you are wishing to take advantage of flexible spending, please complete the form that is attached to the plan information documentation. If you are wishing to get cards for other members on your plan, please complete the card information form. Harvard Pilgrim – select either the HMO plan or PPO plan, complete the enrolment form and return to HR. Delta Dental – please complete enrolment form and return to HR. Vision Services Plan – please complete enrolment form and return to HR.
Eligibility Criteria – one year Tuition Benefit – this benefit is available to all full-time faculty and staff after the completion of 12 months full-time employment with Franklin Pierce University. This benefit is available to yourself, your spouse and your college-aged student dependents. Please contact a member of the HR Team if you have any questions. Benefits that do not require enrolment Employee Assistance Program – the EAP program is available free of charge to employees. Please check their website at www.theeap.com for further information.
