New Hampshire HMO LP: A traditional HMO plan where you will meet an individual deductible, pay co-pays for office visits and can use a Flexible Spending account. Once you reach your deductible, many services are paid at 100%.

New Hampshire ElevateHealth HMO HSA: Harvard Pilgrim's ElevateHealth High Deductible HMO HSA offers high-quality coverage using a select network of New Hampshire's leading health professionals and hospitals at a more affordable premium than a traditional HMO.
Arizona PPO LP: Supported by the United Healthcare Network, this plan offers high choice and flexibility. It's a traditional PPO and you do not need a PCP. You are subject to a deductible and services cost more when out-of-network. You can use a Flexible Spending Account with this plan.
Arizona PPO HSA HD: Supported by the United Healthcare Network, this plan is subject to a deductible and through participation in an HSA you can offset the cost of the deductible with tax free savings. After you reach your deductible, many services are paid at 100%.
